Our example tag includes the following scuba diver medical and information:

• Diver Name
• Date of birth
• Blood type
• Drug Allergies
• Emergency contact name
• Relationship to diver
• Emergency contact phone number(s)

  The base tag comes in many popular colors including blue, green, red, yellow, pink, and black.  The nameplate is riveted to the webbing with four stainless steel rivets.  

  Strap is 9 inches long by 2 inches wide.  Loops around most BC straps and attaches with Velcro tabs sewn on each end – if your BC strap is extra wide, we can make the tag longer but very narrow straps are hard to accommodate.
